SOCIAL STYLE DANCE LLC
DANCE LESSON ENROLLMENT AGREEMENT & RELEASE AND HOLD HARMLESS
1. Your place for all group classes, lessons, and events is held only once payment has been
received. No lessons are scheduled, and no place is held for a group class without payment in full.
Group classes and events have set dates and times, and have a limited capacity. Therefore, registration for any class or event can only be used for the class date(s) or event that it was purchased for.
2. There are no refunds or credits for purchased lessons, group classes, gift certificates, or events. No exceptions. Private lessons, group classes, and events are non-transferrable. Private lessons expire one (1) year from purchase date.
3. Private lessons are scheduled first come/first served as studio and instructor availability
permits. All lessons are approximately 50 minutes. Social Style employs multiple
qualified teachers; a specific teacher is not guaranteed for any lesson, class, or event.
4. Requests for cancellation or rescheduling of any private lesson must be received
on or before 24 hours prior to the scheduled lesson or it is forfeited, no exceptions.
5. Group Classes are held as scheduled barring forces of nature, or fewer than 2 couples
in attendance. If a force of nature causes a cancellation, the group class session will
be extended to still hold the correct number of classes in the session. If only 1
couple arrives for the class, they will be given an opportunity to attend a different class
instead of the class they were initially going to attend. You are welcome to attend a
group class without your partner, if they are unable to make it. However, we cannot allow
a “fill-in” – where another person comes in place of your partner. Only people who are
registered for the class are able to attend.
6. Social Style Dance, LLC makes no representation or warranty that any student will
improve, show progress or become proficient in any dance as a result of the lessons and
events participated in by the student. No results are guaranteed by Social Style Dance,
LLC.
The undersigned, being an adult resident of the State of Wisconsin, certifies the following:
The undersigned understands and acknowledges that any physical activity; including
participating in dance lessons, could involve risk to the student, including injury and trauma,
even when the Student is carefully supervised and instructed, and the undersigned
acknowledges and agrees he/she is physically capable and healthy enough to engage in dance
lessons with Social Style Dance, LLC. Student accepts all risk associated with the provision of
dance instruction by, and the participation in related dance classes and events with, Social Style
Dance LLC.
Notwithstanding the risks associated with engaging in dance lessons and related events, the
undersigned, and on behalf of his/her heirs, successors, executors, and assigns, hereby fully
releases and forever discharges and holds harmless Social Style Dance LLC, and any of its
affiliates, successors or assigns, and its present and former employees, members, officers,
directors, and agents, from and against of any and all claims, actions, rights, penalties,
damages (whether specific, general or punitive), losses, suits, costs, liabilities, causes of action,
or expenses (including attorney’s fees whether or not litigation is commenced), imposed upon,
incurred by, or asserted against Social Style Dance LLC and arising out of or relating in any way
to the undersigned’s participation in dance lessons and/or related classes and events with
Social Style Dance LLC.
It is the specific intent of the undersigned that this release be as broad and as general as the
law permits.
The undersigned makes this release and hold harmless knowingly and voluntarily, with full
knowledge and without duress or coercion.
This agreement shall be construed and enforced in accord with the laws of the State of
Wisconsin.
Studio Policies:
By entering these premises you acknowledge and accept that the nature of dance instruction
involves close proximity and physical contact with instructors and other students.
No video, audio recordings, or photographs are to be taken by any student or visitor to the
studio. By entering Social Style Dance Studio you consent to the use of your image, voice, or
both in any video, photograph, or audio recording for fundraising, advertising, publicity, or any
other purpose on behalf of Social Style Dance Studio, LLC. And you waive all claims to
compensation or damages based on the use of aforementioned video, photograph, or other
recording. You also waive all rights to inspect or approve the finished product. This is perpetual
and may not be revoked.
**The undersigned agrees that any and all additional purchases of lessons, classes, parties or
any other service or product from Social Style Dance, LLC will be subject to the current terms and
conditions at the time of the new purchase.**
IN WITNESS WHEROF, the undersigned has executed this Release and Hold Harmless as of
the date this was completed.
By purchasing from Social Style Dance LLc, I confirm that everyone registering for lessons or classes at Social Style Dance Studio, LLC have read, understand, and agree to all terms listed and cannot dispute them.
